We collect communication information such as patient name, phone numbers, address, and benefit information. This information is provided to us by the patient and/or by the patient's insurance company.

How do we use the information we collect?

We use this information to provide services to our patients, to schedule appointments, to recognize them when they contact us, to bill their insurance company for them.

What information do we share?

We share information only with the patient's insurance company, for the sole purpose of billing their claim for them. We do not share SMS opt-in information.
